JaxCare is a community-supported health plan for the working uninsured. It is available to eligible employees of Duval-County businesses. JaxCare provides a full benefit program to low-wage uninsured workers—a group that is typically underserved.  Based on a managed care model, JaxCare provides members with access to primary care, specialty care, in-patient and out-patient hospital care, diagnostic services, pharmaceuticals and much more.

JaxCare’s public/private partnership model provides the structure and strength of a community-wide initiative to facilitate access to health care services. This approach offers multiple benefits: business-government partnership; community investment and accountability; greater geographic coverage; more consumer choice; and ease of expansion.

JaxCare Health Plan Eligibility Requirements

To be eligible, members must meet the following criteria:

• Age 19-64

• Duval County resident

• Employed for at least 90 days

• Work at least 15 hours per week for the participating employer

• Uninsured for the past six months

• Not eligible for government sponsored coverage programs (e.g., Medicaid)

• Family income at or below 200% Federal Poverty Guidelines (see chart below).
